Whether you are in your home, at a skilled nursing or assisted living facility, Vital Hospice keeps you comfortable and pain-free with a team of experienced and compassionate caregivers. We create an individualized care plan tailored to fit your personal needs. Our hospice care team provides everything from registered hospice nurses, experienced aides, financial and spiritual counseling, and any necessary medical equipment. Your care plan is designed so you can live comfortably, in your own home or nursing community, surrounded by your loved ones.
Hospice Registered Nurse Visits
Our registered nurse visits are scheduled to accommodate to your personalized care plan. Nurses will come to your home to administer professional care to make sure you are comfortable. Your professional care nurse is continual and is scheduled throughout the rotation of care from your nurse aides, volunteers, social worker, and spiritual counselors.
Pain and Symptom Management with Medication
Your pain and symptom management will occur through professional assessments and communication with you and your loved ones. Vital Hospice implements a continuous check on your comfort and pain management providing the right medications at the right time.

Nurse Aide Visits – Scheduled to aid with personal care: bathing, feeding, dressing and other duties
Hospice Team Visits – Hospice counseling, program implementation, insurance coverage, support and follow-up visits
Volunteer Visits – Scheduled for companionship, errands and light duties
Bereavement Services – 13-month bereavement services post-death to all family members and loved ones
